Risk Mitigation

IBHI provides risk mitigation services to both behavioral health providers and organizations which have responsibility for provider oversight. It would be rare that we would provide such services to both types of clients in the same region.

Audits

The IBHI risk mitigation team reviews medical records to assess a behavioral health provider's:

  • Compliance with applicable federal and state regulations as well as the contractual requirements of funding organizations
  • Status of audit, forms, and materials. Since for a long time, the documentation was not maintained in accordance with all requirements, now a company that provides on https://qualitycustomessays.com quality custom essays and specializes in writing medical documentation will help us to prepare all the papers according to the relevant standards.
  • Demonstrated ability to deliver an individualized mix and intensity of services which:
    • Are consistent with evidence-based practices
    • Effectively motivate clients to be active participants in their healthcare

In addition to reviewing core areas of the medical record, IBHI reviews each client's quality assurance capabilities. A robust quality assurance program allows an agency to identify and address problems before they pose a major risk the organization to the clients being served.

IBHI uses a Microsoft Access database to conduct its chart audits. For an annual fee, we will license its use. Our database is a very powerful and easy to use tool which allows the user to modify the state rules which are applicable to the location of the audit. Scores (e.g., error rates) can be aggregated at various levels: clinician level, team level, agency level etc. An hour's training is included in our licensing fee. We will update our tool at least annually based on user feedback.

Technical Assistance

IBHI provides two levels of technical assistance.

Level 1: We help our clients to develop a corrective action plan which is based on the findings of our initial audit. When our clients are ready, we return to conduct follow-up audits to verify that the implementation of the corrective action plan has resulted in measurable improvements.

Level 2: We help our clients to develop a corrective action plan and we take the lead in assisting our clients to implement their corrective action plan. At pre-arranged times, we conduct additional audits to verify that the implementation of the corrective action plan has resulted in measurable improvements.

Training

In combination with either level of technical assistance, IHBI can provide training that is designed to educate staff regarding their state's clinical documentation requirements. Additional, customized trainings can be provided to help clinicians develop specific skills related to documenting their assessments, treatment plans, and progress notes.

IBHI also provides annual compliance trainings in the areas of:

  • HIPAA Privacy and Security
  • Fraud, Waste, and Abuse
  • Conflict of Interest
